Where:    Behrens Park, 493 Oaklawn Ave., Elmhurst

Time:       On-site registration 8:15-9:55am - walk begins at 10 am sharp

Entry Fee: $20 (pre-registration) $25 (event day)

In person: Pre-register in person on Saturday, September 20, 2008 from 12-3 pm at the Center for Speech and Language Disorders, 606 N. Michigan, Elmhurst, IL (Crestview and York Rd. behind the shopping center).

By mail: Download and mail your Registration Form, along with your check to the address below.  (Must be received no later than September 23rd.)

By fax: If paying by credit card, download and fax your Registration form to (630) 530-5909 no later than September 23rd.
